Bug fixes

• Improvements to "All must agree" workflow logic

• "All must agree" workflow events now require a specific transition to be identified in the configuration that represents an agreement. Fixes an issue where a workflow state could be exited with many options instead of one, which could cause problems in ascertaining which action constituted agreement.

• Updates to the activity log to ensure all comments for All Must Agree workflow events are captured correctly.
